Funny thing, I'm already tinkering with a GW heroic / outlast deck:http://www.mtgvault.com/dedwards/decks/striving-to-be-a-hero/Hopefully this can give a few ideas.
Choose to keep one or the other, then replace the other with either Rancor; Lighting Bolt; or Vines of Vastwood.
Huntmaster and Immerwolf don't play well together. Huntmaster wants to flip as often as possible, Immerwolf stops him from doing so.
I'd replace Blade of the Bloodchief with Lashwrithe. It may cost more mana, but I. Prefer an immediate boost over a gained boost, especially against an opponent's wrath effect.
For fast self mill: Mesmeric Orb :P
Mirrorweave is an interesting combo piece. I would have gone with Followed Footsteps + Paradox Haze myself. Another one I might have used is Right of Replication. Trying to figure out why Vault says this deck is banned in Modern. It's Green Sun's Zenith. Sadly that card is currently banned in Modern. Some possible replacements include Summoner's Pact; Fauna Shaman; Chord of Calling; Primal Command; and Tooth and Nail. Commune With Nature is another option, though it only looks through the top 5 cards of your library.
With a fair amount of your creatures costing a bit more mana than I'd like, I suggest running a full set of Elvish Mystic.A set of Hardened Scales would be cool. Bolster is another good source of +1/+1 counters.
Quest for the Holy Relic?
Holy Day specifies combat damage. Ethereal Haze stops all damage by creatures both inside and outside of combat (*cough*Vexing Devil*cough*). Both last until the end of the turn they get cast in.
Interesting build. With a bit of tweaking (Cranial Plating), this could be even better. Ring of Thune might be worth considering, free +1/+1 counter every upkeep and vigilance. I personally prefer Ethereal Haze over Holy Day.
Most of my green aggro decks use 4x Rancor; 4x Mutagenic Growth; 3/4x Vines of Vastwood (when possible). I find they help towards consistent wins.
Rancor
Howl of the Nightpack might be a card that interests you.Blanchwood Armor and Traproot Kami are another two interesting cards that care for the number of Forests out. I'd also swap out Elvish Mystic for Search for Tomorrow.
Most consolation decks I've seen are GB, most probably to use Eidolon of Blossoms and Courser of Kruphix (Boon Satyr too?). BW is an interesting route, but I think you will have a similar problem I'm having with my GW Heroic deck, finding the correct balance of creatures to non creatures. You may want to include a few more non enchantments for early game defence. I suggest Gods Willing and Bile Blight.
Not seeing the point of Selesnya Sanctuary in here.

This has happened a lot: Turn 4; "if you don't kill the Guard or counter my Gond, I win." Though in the longer matches, I generally win off Sprout Swarm.
I have to share this: I have a more spell oriented BW token deck that I run from time to time and last time I ran it I had a surprising win against RDW. The opponent had Eidolon of the Great Revel out, I was on 7 life with 3 spirit tokens and 1 Honor of the Pure, he on 13, but with no cards in his hand and only the Eidolon. He draws a burn spell, hits me for 3 (me 4, him 11) and passes. I top deck Spectral Procession, worried about every card I play potentially killing me, then reread the card and smiled. Because the Procession technically costs 6, even though I only paid 3 white, the Eidolon didn't do damage to me :) I swung for 6 and play the Procession (me 4, him 5). He drew a land and passed. I swung for lethal. Conclusion: even with RDW's awesome new tool, BW tokens is still an awesome deck :P
