Here is some tips.
out with Grave Titans Completly it is a vampire controll so in with Anowon instead
out with 2 Duress and 1 Inquisition of Kozilek to much of those IMO
in with 2 Nirkana Revenant instead. it is going to boost your mana
Sadistic Sacrament is good but it is to slow IMO put in either 2 Grasp of Darkness or 2 Guul draz Assassin
out with 1 Memoricide and out with Kalastria and in with 2 Diabolic tutor
you are not going to need 25 swamps. 2 is good for this deck
20 Swamps and 4 Verdant Catacombs

my ideas. Hope it will help

I agree that you're running too few vampires to call this a vampire control deck. You may have a good early start but you don't have any REAL mid-game power. You might say that Grave Titan is mid-to-end game power but what happens when someone double lightning bolts him? Or maybe they'll attack with a green 3/3 creature and use Giant Growth. Maybe they'll hold a Deathtouch blocker... Maybe the blocker gets boosted to 6/x .... What I'm saying is, you're missing an army to back up the Grave Titan. Sure, making people sacrifice creatures helps but a lot of decks run lots of creatures, burn, counters, or mill. You won't be able to truly keep up with them and that will be a problem.

You should consider reducing to 20 land as well. The usual land recommendation is 33% and you're beyond that. If you're worred, run Nirkana Revenants. They are vampires as well so it gives you a more vampire-focused control deck.

Think about But Think is 20 lands is not enough, whatabout 24 lands and 1 nirkana

Nirkana makes any mana you tap worth 1 extra. With 2 Nirkana's out, tapping 1 swamp provides 3 black mana. Dropping down to 22 lands would be preferred over dropping to 24. You could get 3 Nirkana's Revenants. Another idea is to drop to 20 lands, add vampires from that, and then swap disfigures out for Urge to Feeds. You can tap any vampires, including ones with summoning sickness with the Urge to Feed's effect.

I'd say take out wurm, and put in another Kalastria.

I'd also say take out 1 Memoricide 1 Feast of Blood and 1 Duress and put in 3 Urge to Feed.

You should be ok with 22 land also, so like 18 Swamp and 4 fetchers.

I have a Legacy Mono Black Control and a Standard Mono Black Control the Standard one is alot like this.

Standard: http://www.mtgvault.com/ViewDeck.aspx?DeckID=145209

I would really take out Sorin. He's just not as good as you might think, and overall will just be mana burden in your deck the majority of the time. =/

Drana.
She's a greatly over-looked vampire with easy kill potential.
Seems to fit right in with your build. I'd bin a disfigure or two for the Drana's.

so heres what im thinking captivating vamp is amazing if and only if you have five vampires out when he is otherwise hes an kinda worthless just sayin i would run way more little vamps to play to your deck like hexmage and the one drop that lets you scry by sacking a creature i mean with grave titan you can basicly cycle through your deck to get what you need and i agree with surewhynot's comment drop sorrin hes not doing really anything in your deck but mostly run more vampires if your going to run captivating cuz other wise hes not nearly as good as he could be. plue hexmages make plainswalkers cry just sayin
