Gatherer Card Rulings for Fiend Hunter:
9/22/2011: If Fiend Hunter leaves the battlefield before its first ability has resolved, its second ability will trigger and do nothing. Then its first ability will resolve and exile the targeted creature indefinitely.

This rule is also true for Journey to Nowhere and Oblivion Ring.
Obviously, Brittle Effigy, Archon of Justice, Venser, Karn, Iona's Judgment, and Path to Exile already permanently exile cards.

I.e.: You play Fiend Hunter. His first ability goes on the stack. Before it resolves, you target it with Unsummon. Unsummon resolves returning Fiend Hunter to your hand, triggering his second ability placing it on the stack. The second ability resolves first and so does nothing. Then the first ability resolves, permanently exiling the targeted card.
