FYI, Whip of Erebos and Fate Unraveler are a good combo too :) Also adds 2 black for devotion.
Just a suggestion to use Centaur Healers to gain life fast before you play the worm or maybe Elixir of Immorality. Also why no mana ramp?
the reason I was saying is you have barely ANY triggers in the deck for Evolution. So with that in mind getting 2 +1/+1 counters on Experiment One to regenerate is pretty hard to do. I might also suggest Krasus Incubation and Bred for the Hunt. Bred for the Hunt is cheaper than Fathom Mage and harder to get rid of. Also, 2 copies of Forced Adaption and Burst of Strength should be at least 3 of each IMO and according to the numbers. If you ever hope to draw them.

Drop Experiment One and Shamble Shark add in Forced Adaptation and Burst of Strength.
Loos the Carnival Hellsteed, and Chaos Imps. Add in Splatter Thug and Thrill-Kill Assassins. Drop 1 mountain and 1 swamp and add Seismic Stomp.
Get rid of Unknown Shores all together. 22 land is fine. you need to run Door of Destiny for slivers. Remember all your slivers can tap for any color mana with the Manawft Slivers.

I had this deck built. It's pretty fun. but I ran x2 Immortal Servitude instead of the Grim Return. Also, Viskopa Guildemage is great with it's second ability and the combo.WHY is your sideboard over 15 cards?

Try some Forced Evolution auras in the deck it puts a +1/+1 counter on the creature each upkeep, works great on the Fathom Mage to draw 2 cards a turn, also Corpsejack Menace and Vorel of the Hull Clade help make lots of +1/+1 counters.
It's pretty close to one. Still that's a bit too much land your very likely to hit mana pockets going over 22.
I'd replace Soldier of the Pantheon with Phalanx Leader to pump up the tokens you get.Spirit of the Labyrinth is a side-board creature. Also, drop Blind Obedience, better replaced with x3 Imposing Sovereign.You may want to run a few Smites as well.Drop x2 Plains your running way overboard on land, 22 is standard for white weenie.Nice concept all in all.
